Along with the duly-filled application form, you need to submit documents related to your identity and address. As proof of identity, you can submit any of the following – voter ID, Ration Card, PAN, Driving License, Passport, AADHAR, or any ID card issued by the Indian Government/any Government ID Proof for NRIs.

You also need to disclose your – annual savings, medical cover, medical records, family doctor/treating doctor name and address with contact details,  and assessments, and contact of family members, preferably an adult child who may sign agreement on behalf of parents in case of any medical emergency.

Move to Alpha

Welcome to the Alpha Elite Assisted Living Home Family. In the final stage of moving in, you must bring your essential items like medicines, personal pieces of stuff, gadgets, and clothes. At Alpha, the suites are fully furnished, hence, you need not bring your furniture along.
